Employer Injury Management Program Checklist



    Employee Communication
  1. Job descriptions and job function evaluations available for each job
  2. Employees provided with explanation of WC rules and benefits
  3. Workers informed of polices related to fitness for duty, e.g., substance abuse
  4. Policies for alternative work/modified duty for lost time injuries shared with employees
  5. Injury reporting procedures clearly communicated to all employees
  6. Employees provided with names/addresses of designated or recommended medical providers

    Policies & Procedures
  7. Require that all employee injuries be reported immediately, no later than 24 hours
  8. Require all injuries to be reported in writing and signed by the the injured worker
  9. Stipulate that the injured worker's supervisor be informed within a specific timeframe
  10. Designate and train WC claims coordinator(s) in claims management procedures
  11. Arrange for backup procedures when designated person is unavailable
  12. Submit WC insurance claim within 24 hours of the injury
  13. Provide complete information on the First Report of Injury
  14. Communicate suspected fraudulent claims to the WC insurance adjuster
  15. Implement accident investigation procedures which may require photos and physical evidence
  16. Pursue alternate/modified duty options for each lost time injury
  17. Require upper management notification when lost time exceeds 2 weeks
  18. Supervisors aware of their responsibilities related to injury management

    Claims Management
  19. Designated (or recommended) medical provider(s) part of WC insurer's PPO
  20. Ongoing, medical services evaluation procedures
  21. Job descriptions/functional evaluations given to treating physician and WC adjuster
  22. Medical provider aware of return to work policy
  23. Treating physician contacted within 24 hours of a lost time injury
  24. Injured employee with lost time injury contacted within 24 hours
  25. Effective followup until Release to Full Duty
  26. Ongoing, timely communication with WC adjuster
  27. Telephone log of contacts kept
